We work with people aged 18-60 who are learning disabled and on the autistic spectrum. You and Me offer services to enable independent living based on activities.
We know our loved ones do not always want to join us for family events or holidays and sometimes we need a break. We can plan with you regular or a one off live in respite care. This should allow you the opportunity of knowing your loved one has not had the disruption of moving out of the family home.
The aim would be to have as little disruption as possible to the weekly routine, provide support workers already known to the family. We would always complete an assessment of need and provide the family with an agreed care support plan. We will always support and maintain the continuity of care from other services, for example, district nurses, GP, clinics and pharmacies.
Its all about activities that promote independence, help develop new skills, confidence and achievement. That's why we partner up with local community facilities to ensure people get the most out of life.
